10/31/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to home to conduct individual session with Chevelle. Chevelle appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Chevelle discussed with QMHPA her afternoon was going well. Intervention: QMHPA began the session by encouraging Chevelle to strength her communication skills when it comes to expressing her thoughts and feelings. QMHPA followed up with Chevelle regarding the communication skills with her family members. QMHPA discussed desire to have what she perceived as traditional communication skills and how that might be harmful to her emotional health. QMHPA educated Chevelle on the benefit of compartmentalizing her communication skills and being more discerning about how she communicates. QMHPA suggested for Chevelle to identify the traits of beneficial communication skills and harmful communication skills. QMHPA actively listened as Chevelle listed the traits of both types of communication skills. QMHPA asked Chevelle to list people in her life that fell into both categories. QMHPA encouraged Chevelle to work on building the beneficial communication skills before repairing the harmful ones to assist with increasing positive social skills. QMHPA advised Chevelle that some of the communication skills may require purging in order her to maintain her emotional and mental health. QMHPA advised Chevelle that certain communication skills may cause more harm than good. Response: Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her by QMHPA. Chevelle stated that her communication skills with her family continue to be a concern for her especially now because all she feels is anger and resentment about her life and those in it. Chevelle would not elaborate. Chevelle referenced her inability to let go of some of the negativity in her life that may be harmful. Chevelle admits that she cannot seem to find a way to repair her life or those things that are troublesome. Chevelle referenced being unhappy within her family but wanting to be able to forgive and make amends to those she feels has made her feel unhappy so that she could proceed with the rest of her life. Chevelle discussed not being able to move past things from her past. Chevelle participated in the exercises willingly. Chevelle demonstrated knowledge of healthy and unhealthy communication skills. Chevelle was verbal while relaying instances of perceived dysfunction in her interpersonal relations of her family. Chevelle listened actively, fully participated and gave feedback throughout the visit. Plan: QMHPA will continue to engage Chevelle in more role-playing activities assist her with learning how to respond composedly in stressful situations that may occur (1-2 times weekly).
